When Alarm Systems Overreact

Anxiety is a normal and important response to threats. In anxiety disorders, this alarm system is overactive – it responds even without real danger, too strongly, or for too long.

Neurobiologically, the networks for threat detection and evaluation are dysregulated. This leads to intense physical symptoms and avoidance behavior, which increasingly restrict daily life.

Anxiety Is Not Dangerous – But Burdensome
Physical symptoms of anxiety can feel threatening but are not dangerous. The key is breaking the cycle of anxiety, avoidance, and increasing restriction.

Types of Anxiety Disorders

Panic disorder with recurrent panic attacks
Social phobia (social anxiety disorder)
Specific phobias (e.g., heights, flying, animals)
Agoraphobia (fear of public places or crowds)

Panic Disorder and Social Anxiety

Panic disorder involves sudden, intense episodes of fear with symptoms such as racing heart, shortness of breath, or dizziness. Often, fear of the next attack develops.

In social phobia, fear of negative evaluation dominates. This can lead to strong withdrawal and significant distress.

Why Careful Assessment Is Important
Anxiety disorders rarely occur in isolation. Only when all relevant factors are recognized can a suitable and effective treatment be planned.

Treatment

Psychotherapy

Cognitive behavioral therapy with exposure techniques is the most effective treatment for anxiety disorders. Confrontation with anxiety-provoking situations is done gradually and controlled.

Medication Support

For severe symptoms, antidepressants (SSRIs, SNRIs) can be used as support. They do not replace psychotherapy but can facilitate the start.

Combined Treatment

In many cases, combining psychotherapy and medication is the most effective approach.
